At Boulder Natural Health, we see women, men, children and the elderly, and treat both acute and chronic health conditions. Patients can make an appointment either by calling the office at 303-960-3920, or by sending an email to the clinic via our contact page.
In most cases it is possible to schedule an appointment within 2-7 days, and often patients can be seen the same day in urgent situations.
We ask that new patients fill out all of the required intake forms before your scheduled appointment. You can download these forms at this link: Download Forms. We also ask that you:
- Bring a copy of your most recent medical records, including any laboratory reports that are available.
- Bring in any supplements or medications that you are currently taking.
